Function Over Fractions: the true measure of rehab

When does quantitative measurement become more important than qualitative measurement?
The year was 2007. I was still studying physiotherapy when my lecturers were already discussing quantitative and qualitative measures in clinical practice.
Now, after more than 15 years of experience in healthcare, I have come to understand something that has become very clear to me:
Patients need quality of movement more than simply quantity of movement.
My clinical goal has always been function.
Improving range of motion, strength or mobility is important. But the question I have learned to ask over the years is:
“Does this measurement have functional quality?”
We have excellent quantitative tools that help us to:
- compare progress;
- document outcomes;
- communicate data between professionals;
- support scientific research;
- monitor therapeutic goals.
All of this is extremely important.
However, isolated numbers do not always represent function.
Imagine a patient who achieved 90° of elbow flexion. An excellent quantitative outcome.
However, when we observe more closely, we notice significant muscular compensations. Once the accessory muscles are inhibited, those 90° of elbow flexion no longer exist.
This leads to an important reflection: What was the real functional benefit of those 90°? Perhaps none. Because the movement was not functional, not efficient, and did not provide independence for the patient.
Now imagine another scenario.
The goal was to achieve 90° within a certain timeframe, but the patient improved by only 20°. Quantitatively, the target was not achieved. But qualitatively, those 20° occurred without muscular compensation and allowed the patient to open and close a door functionally.
In this case, there was real progress.
Quantitative measurement is essential for comparison, documentation, research, and tracking objectives. But it is qualitative analysis that reveals the true functional impact within the context of the patient’s clinical diagnosis, prognosis, and daily life.
In the end, perhaps the most important question is not: How much did the patient gain?
But rather: What has the patient regained the ability to do?
